Physical Examination

Each student who wishes to participate in sports must have a physical examination every two years. A physical card must be on file in the school office prior to any practice or participation. Make sure your athletic card status is up to date! If you need a White Physical Card (Physician Signature) or a Green Alternate Card (Parent Signature) they can be requested from the school office.
